*Licensed Practical Nurse (LPN) - Primary Care Medical Practice*

*Responsibilities:*
* *Patient Care:*
* Provide routine care to patients, including monitoring their health status and addressing basic medical needs.
* Assist with patient assessments, vital sign measurements, and health history documentation.
* Administer medications and treatments as prescribed by clinical providers.
* Ensure patient comfort and safety during clinic visits.
* *Collaboration:*
* Work closely with physicians,Licensed APRNS, and other healthcare professionals to coordinate patient care.
* Communicate effectively with patients and their families, explaining treatment plans and answering questions.
*Documentation:*
* Maintain accurate and up-to-date patient records.
* Document observations, interventions, and patient responses.
* Assist with charting and record-keeping.
* *Procedures and Education:*
* Assist physicians and Licensed APRNs during minor procedures.
* Educate patients on medication administration, wound care, and preventive measures.
* Provide health education to patients and their families.
* *Clinic Operations:*
* Manage patient flow within the clinic.
* Maintain exam rooms and equipment according to infection control standards.
* Collaborate with non-licensed nursing staff.
* Other Duties as assigned
*Qualifications:*
* *Valid LPN License:* Must hold a current LPN license in the state of practice.
* *Clinical Skills:* Proficient in basic nursing procedures, medication administration, and patient assessment.
* *Communication:* Excellent verbal and written communication skills.
* *Compassion:* Demonstrates empathy and compassion towards patients.
* *Team Player:* Works collaboratively with colleagues and contributes to a positive work environment.
