ESSENTIAL JOB FUNCTIONS: Provide ongoing service coordination for eligible children as the assigned service coordinator.
Facilitate and participate on Multidisciplinary teams – Developmental evaluations, Individual Family Support Plan (IFSP), Transition.
To assist in determining eligibility, To assist with reviewing and assessing interventions and treatment on a regular basis, To assist with transition from IDEA Part C Promote and support the IFSP team’s capacities and competencies to identify, obtain, coordinate, refer, monitor and evaluate resources & services to meet the family’s needs.
Interview families to obtain information on family composition, dynamics, routines and activities, formal and informal supports, concerns, priorities and resources relevant to the child’s development, and present levels of development that influence intervention and treatment.
Collaborate with families by developing timely and extensive Individualized Family Support Plan (IFSP) and implementing, through referrals to community agencies.
Communicate IFSP recommendations to other professionals and team members involved in IFSP and coordinate IFSP Reviews and updates, as necessary, at minimum every 6 months.
ommunicate with families and IFSP team members providing follow-up reminders to promote and facilitate timely developmental evaluations.
Coordinate with Service Coordinator Supervisor on all children transitioning from Part C, Prepare case notes and documentation, maintaining accurate file documentation.
Maintain billing practices for Children’s Medical Services Targeted Case Management services Preserves the integrity and confidentiality of protected health information of Easter Seals clients.
Understands, acknowledges, and implements Easterseals privacy policies involving the disclosure of protected health information of Easterseals clients.
Demonstrates a strong commitment to self-development.
Maintains a clean, safe, hazard-free work environment.
Completes all assignments and responsibilities, is flexible and a team player.
Acts in accordance with the Easterseals behavioral expectations, mission, vision and values.
EDUCATION and/or EXPERIENCE: Bachelor’s Degree or higher from an accredited university with an emphasis in one of the following areas: psychology, social work, health education, interdisciplinary sociology, early childhood, child development, or special education, or an equivalent degree based on transcript review.
One year of professional experience with young children and families in teaching, case management or counseling role with individuals with special needs and/or developmental delays of any age in case management role.
Florida Driver’s License with daily access to a reliable and insured vehicle.
Ability to secure and retain a Medicaid Provider Number KNOWLEDGE, SKILLS AND ABILITIES: Develop a strong knowledge base and stay current on job related issues and trends.
Community resources, Child development and disabilities, Applicable confidentiality and other privacy laws Participate actively in departmental meetings, trainings and education, as well as quality process.
Demonstrate a commitment to self-development.
Skills in completing tasks correctly and timely.
Ability to travel to family homes or locations selected by families in order to obtain information and complete tasks.
Ability to adjust schedule and meet needs of families.
Provide crisis management assistance as necessary Positive attitude towards individuals with disabilities and the Easter Seals work environment and philosophy.
Willingness to be flexible.
Possess the qualities of integrity, tact, and good judgment.
PHYSICAL CAPABILITIES: Able to receive oral communication via phone or in person.
Able to stoop, kneel, crouch, and reach.
Able to assist with lifting and carrying children weighing up to 50 lbs.
USE OF TOOLS AND EQUIPMENT: Able to operate copier, fax, and computer equipment.
WORK ENVIRONMENT: Employees are required to work in a drug-free workplace.

Job Type: Full-time Pay: $44,000.
